Forms
Here are the most frequently used forms that you might need.
- Mortgage Application Form
- Mortgage Application Checklist
- Salary Certificate – To be completed by your employer
- Building certificate for Self-build customers
- Certified costing template – completed by an architect, engineer or building surveyor on behalf of self-build customers
- Self-build overrun overspend template – Completion is required if you have overruns on a self-build property
- Gift Letter - If you have been gifted funds, a site or equity into your account to help with the deposit of your mortgage, you are required to complete this template
- Mortgage Valuation Booking - Book your valuation as part of your new mortgage application. If you are looking to change the Loan To Value (LTV) on your existing mortgage, see here .
- Direct Debit Mandate: All mortgage applicants will be required to complete this form to allow AIB to collect your mortgage repayments by Direct Debit

Forms
Here are the most frequently used forms that you might need.
- Mortgage Amendment Form – This form allows you to request amendments to the Repayment Date and to the Loan Rate on your Current AIB Mortgage.
- Payment Break form – Application form to apply for a Payment Break (i.e. Moratorium or Interest Only) from your mortgage.
- Overpayment Form – If you would like to pay a lump sum off your mortgage.
- Mortgage Overpayments following a Covid 19 payment break. If you have taken a Covid 19 payment break and would like to make an overpayment.
- Mortgage Valuation Booking - Book your valuation as part of your new Top Up or Equity Release mortgage application. If you are looking to change the Loan To Value (LTV) on your existing mortgage, see our Resdential Mortgage Panel page.
- Reduced Repayment Application Form for Alternative Mortgage Repayments - To be completed if you require an alternative repayment applied to your mortgage and your mortgage account is NOT currently being managed with the Mortgage Arrears Resolution Process (MARP).
- Direct Debt mandate: If you would like to change the account your Mortgage is paid from, please print out and complete the Single Euro Payments Area (SEPA)Direct Debit Mandate form.
- Transfer of Title Tracker Interest rate application form: this form should be used when adding or removing an applicant to or from your mortgage loan and/or title deeds and keeping your existing tracker interest rate on your current property.
- Costings template – For customers choosing a top-up mortgage loan for home improvement purposes, your mortgage advisor will let you know which of the following to use:
- Self Certified Costings – To be completed by the customer.
- Certified Costings Template – Completed by an architect, engineer or building surveyor on behalf of the customer.
- Building Certificate for Self-build and Top-up Mortgage customers
- Mortgage Reclassification Form
